Insurance
I am an out-of-network provider. If you intend to use your insurance for reimbursement, I recommend you contact your plan prior to our first session to ask about your psychotherapy benefits. Many health insurance policies provide some coverage for mental health treatment. I am considered an "out-of-network" provider, which means that your insurance company may reimburse you directly for some portion of the fees you have paid to me. I will provide you with a receipt and will also help you receive the benefits to which you are entitled; however you, not your insurance company, are ultimately responsible for full payment of fees.
The law protects the relationship between a client and a psychotherapist, and information cannot be disclosed without written permission.
Exceptions include:
Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
I understand things come up and appointments may need to be cancelled or rescheduled from time to time. I request cancellations 48 hours in advance of our appointment, or you will be required to pay the full cost of the session.
